E-Passport Application

Valid Omang
Passport application to be completed at the embassy (note that the applicant should have their parents' identity details such as Omang number, date of birth, place of birth, and residential address)
2 full-colored passport photos (35*45mm /1.37*1.77in)
US $40 in cash or money order
Note: Applicants are allowed to apply for both passport and Omang; For lost or expired
Omang, passport application will be processed upon receipt of the renewed Omang
Citizenship certificate if applicable
Forms must be filled with black ball point pen
Fingerprints must be captured in black ink

Passport application must completed in person at the embassy by a parent/guardian on behalf of the minor, who should also be present (note that applicant should be able to provide identity details for both parents' such as Omang number, date of birth, and current residence address)
Birth certificate of the minor
Parent’s valid Omang
Proof of citizenship if parent/s were once foreign nationals (naturalization/registration certificate)
2 full-color passport photos
Previously held passport
US$12 in cash or money order
Citizenship certificate if applicable

For children with dual citizenship, the passport validity is restricted to when the child turns 21 years of age (for renunciation purposes).
