  2. Form 3 - Medical Report (Include form 3 for everyone if accompanied)
  3. Certified copy of Passport (include copies of children if accompanied)
  4. Certified copy of Birth Certificates of children if accompanied
  5. Certified copy of Marriage Certificate if accompanied by spouse
  6. Two passport-size photos (not computer photos) (include children’s photos if accompanied)
